• Temperature or Headache
• Cough or Cold or Flu Like Symptoms
• A Sore throat
• Or Shortness of breath

Covid-19 Policy: Please do not attend the practice if you have any of the above symptoms without discussing with Reception staff first.

NOTE: MASKS COVERING BOTH YOUR NOSE AND YOUR MOUTH ARE MANDATORY IN OUR PRACTICE IF YOU HAVE SYMPTOMS.

A NEGATIVE COVID TEST IS REQUIRED UPON ENTERING THE PRACTICE IF YOU HAVE SYMPTOMS.
